Annoying Tool Options Window

Not sure how it's happening, but the annoying "Tool Options" tab keeps appearing in the upper left (see attached image and I can't get rid of it??

Annoying Tool Options Window

Not sure how it's happening, but the annoying "Tool Options" tab keeps appearing

OSX problem

Try closing the tool options tab and then re-opening it.
